PRE-TAX BENEFITS
1. Tax Deferred Annuity:
The tax deferred annuity benefit allows employees to defer a portion of
their income into a retirement account before their taxes are taken out.
This income and the earnings are not taxed until the employee withdraws the
money. Employees have a choice between the following three companies.
Dobson, Lange & Associates, who offer the Guardian Investor and Nationwide
Best of America. Interested employees may call Doug Dobson or Mark Lange at 345-7526.
Sander Ludeman from Edward Jones. Interested employees may call Sander at 385-0305 or toll free at 877-385-0305.
2. Dependent Care:
This benefit allows the employee to defer the cost of day care (child,
dependent spouse, or parent) into a tax sheltered account. As day care
expenses are incurred, the employee invoices the account.
3. Medical Spending Account:
Employees may defer up to $1,000 per year into a tax sheltered account. As
medical expenses are incurred, the employee may invoice the account.
Expenses such as eye exams, eye glasses, dental, etc., which are not covered
by health insurance are allowable.
4.Medica:
Group health insurance is available to MVAC regular (non-temporary)
employees working 20 or more hours per week. Employees are given a choice of
two plans through Medica. Both plans include prescription
coverage. Application must be made within the first thirty days of
employment and coverage begins the first of the month following a thirty-day
waiting period. Further information and a comparison of the plans available
contact Human Resources. Premiums are pre-taxed and are based upon the
employee's age and specific spouse/dependent coverage selected.
5. Cancer, Hospital Intensive Care And Accident
Protection Plan:
Cancer Care: This cancer plan provides extra funds to assist you in meeting the high cost of cancer treatment. This helps with “indirect” or “out of pocket” expenses such as travel, loss of wages, childcare and deductibles that are incurred with a serious cancer illness.
Intensive Care: The intensive care plan is designed to provide additional funds to help meet unusual expenses when you or a member of your family would need to be taken to the Hospital Intensive Care Unit, Coronary Intensive Care Unit, or Neonatal Intensive Care. It pays $600 - $1000 per day regardless of other insurance or actual charges.
Accident Plan: This program protects you in 4 ways:
Death Benefits...Up to $100,000.
Hospital Benefits...Up to $1000 the first day, $200/day thereafter.
Lump Sum Cash Benefits Paid to you per Injury...Up to $50,000.00 per injury.
Emergency Room and Chiropractic benefits due to accident.

MUTUAL FUNDS:
Employees interested in investing in Mutual Funds may do so with a payroll
deduction with either of the three companies listed below. Mutual funds are an
after tax investment.

LINCOLN NATIONAL
UNIVERSAL LIFE:
This policy is available through our Aflac agent, Gene Wesely.
Spouse and children may also be included with this policy. The employee pays
the entire premium.
PERSONAL COMPUTER
PURCHASE PLAN:
The Employee Personal Computer Purchase Plan is designed to assist MVAC
employees with the purchase and financing of a personal computer so that
they can learn, and gain experience by working with a PC away from the
office, outside of normal working hours. All regular non-probationary
employees of MVAC are eligible to participate. All those who elect to
participate must comply with the requirements and provisions of the Plan as
set forth by MVAC. MVAC will finance the employee's purchase of approved PC
hardware and business software up to $2,000 from an approved vendor through
a no-interest loan.
BANKING SERVICES:
Interested employees may join the Minnesota Valley School Employees Federal
Credit Union. They offer a wide variety of services including saving and
checking accounts, loans, special share accounts and direct deposit.

EMPLOYEE ASSISTANCE PROGRAM:
This service is available through Riverview Clinic for employees working
twenty or more hours per week. The Employee Assistance Program is a
problem-solving resource where a professional counselor will assist you in
assessing your situation, help you look at options, choices, or assist you
